Cassius Photography, Film, Audio, Quotes and Case Studies Consent Form

We like to learn from and share the experiences of real people who are using our services as it helps to demonstrate the difference it is making and will enable us to continually improve. By completing this form, you give us permission to contact you. Thank you for your help.

The Cassius service (made up of Suffolk County Council, Alcove, Rethink Partners and Medequip) would like to take photographs, film, audio of people and gather quotes/case studies of our projects. We might want to use these for promotional purposes, such as our printed publications, social media, websites, external media and film. We may also contact you to participate in surveys or focus groups.

To comply with the Data Protection Act 2018 which has replaced the Data Protection Act of 1998 we need your permission before we take and use any film or pictures and quotes of/from you (or the person you are consenting on behalf of). If photography or filming of groups is organised, and individuals could be easily identified our project workers must find out whether you want to be included. We will use images for a total of three (3) years with permission.

Our promise to you
The Cassius service promise to use photographs in a responsible way, and in appropriate contexts. We may use group images with very general labels, such as ‘older people using tech’.

We will store your information securely. If you change your mind, you have a right to amend, delete, and request access to your information by emailing liz@rethinkpartners.co.uk.

This form is to be used when anyone involved with Cassius and any of its partners - through our work is to be photographed or filmed. Consent is sought by the project worker. Cassius will always brief you as to where the photographs and visual images will initially appear. Photographs might be used in print or broadcast media, social media or on websites. However, where the finished articles and photographs are the property of the media outlet, we cannot control use by non-contracted third parties.
