Photography, Film, Audio, Quotes and Case Studies Consent Form

Rethink Partners and its direct partners, would like to take photographs, film, audio of people and gather quotes/case studies of our projects. We might want to use these for promotional purposes, such as our printed publications, social media, websites, external media and film.

To comply with the Data Protection Act 2018 and the General Data Protection Regulations (2018) we need your permission before we take and use any film or pictures and quotes of/from you (or a child or vulnerable adult that you are consenting on behalf of).  If photography or filming of groups is organised, and individuals could be easily identified our project workers must find out whether you want yourself (or your child/vulnerable adult) to be included.  We will use images for a total of 5 years with permission.

Please answer the questions below and sign and date the form where shown.

I am happy to have photographs or filming taken/provide a quote or case study
