This session explores how the Royal Borough of Greenwich, Oxleas, the South East London Integrated Care System and partners, including Rethink Partners, have come together to design and deliver DHACT (Digital Health and Care Technology) as a live, operational example of integrated care.
One year on, the programme demonstrates how strong partnerships, shared purpose and a clear leadership vision can translate national ambitions into meaningful change for residents. Through real stories, joint commissioning, aligned pathways, cultural transformation and emerging impact, the session examines why integration is achievable and the practical conditions that enable it, from trust and governance through to data, technology and workforce readiness.
Aimed at local authorities, commissioners, ICS leaders, providers and transformation teams, this webinar provides grounded insight, honest reflections and transferable learning for systems working to move joined up health and social care from strategy into everyday practice.
